Development of New Drugs

Creating a new medicine isn't as simple as mixing chemicals in a lab and hoping for the best. Drug development is a lengthy process that can take over a decade and costs millions of pounds.

Most new drugs start their journey in pharmaceutical laboratories where chemists synthesise compounds, though some originate from natural sources like plants. Before any human gets near these potential medicines, scientists must prove three crucial things: efficacy (does it actually work?), toxicity (is it safe?), and dosage (how much should people take?).

Preclinical testing happens first, using cells, tissue samples, and live animals to check basic safety. Only if these tests succeed can the drug move to human trials. Clinical trials follow four stages: healthy volunteers test small doses for safety, a few patients try low doses to see if it works, larger patient groups test different doses to find the optimum amount, and finally a double-blind trial determines true effectiveness.

Think of monoclonal antibodies as incredibly precise biological missiles that can target exactly one specific cell or chemical in your body. Unlike normal antibodies that attack various threats, these laboratory-created proteins are designed to bind to just one target.

You've probably encountered monoclonal antibodies without realising it - they're the clever technology behind pregnancy tests. During pregnancy, women produce a hormone called HCG, and monoclonal antibodies in the test can detect even tiny amounts of this hormone in urine. They're also used medically to locate blood clots and identify cancer cells.

However, their development raises ethical concerns since mice must be injected with diseases to produce the necessary cells. The process also requires inducing cancer in mice to obtain tumour cells. Despite these concerns, treatments like Herceptin for breast cancer have proven highly successful, being both specific to cancer cells and relatively non-toxic, though the cost remains enormous.
